If a big stock or real-estate gain hits in Q3, you should increase the estimate for that quarter to avoid a penalty. When income is lumpy, some use the "annualized income method" (Form 2210 Schedule AI) to match payments to when income was actually earned, rather than splitting evenly.

Pro tip — withholding is treated as paid evenly across the year: withholding from wages/pensions is deemed paid evenly over the whole year, regardless of when it actually occurred. So if you discover a shortfall late in the year and have a W-2, you can increase withholding (adjust your W-4) to retroactively cure the underpayment. A useful move if one spouse has W-2 income.

The underpayment penalty is essentially "interest." Miss estimates and you owe a penalty computed on Form 2210 — in effect interest charged per quarter.
